Streptomycin

Dosing

CrCl Formula

CrCl > 80: 0.5 - 1 g q 12 h.

CrCl 80-50: 7.5 mg/kg q 24 h.

CrCl 50-10: 7.5 mg/kg q 24-72 h.

CrCl < 10: 7.5 mg/kg q 72 - 96 h.

Hemodialysis: 500 mg after dialysis.

Important side effects

Oto and nephrotoxicity. Arachnoiditis, encephalopathy, optic nerve dysfunction, peripheral neuritis, eosinophilia, fever, neuromuscular blockade, respiratory paralysis, paresthesias, vestibular and cochlear toxicity.

Important drug interactions

Bumetanide, capreomycin, carboplatin, cidofovir, cyclosporine, ethacrynic acid, furosemide, lysine, magnesium, nondepolarizing neuromuscular blockers, piretanide, succinylcholine, tacrolimus.

Use for

Tuberculosis. Brucella, Chancroid, Endocarditis, granuloma inguinale, Haemophilus influenzae, Mycobacterium infections, Plague, Pseudomonas mallei infections, Rat Bite Fever, Tularemia, UTI, Whipples disease.

Don't use for

Class

Aminoglycoside.
