CrCl > 50:

immediate-release less than 60 kg, 30 mg po q 12.

immediate-release 60 kg or greater, 40 mg po q 12 hr.

extended-release less than 60 kg, 75 mg po qd

extended-release 60 kg or greater, 100 mg po qd.

CrCl 50-26: less than 60 kg:

CrCl 26-50: mL/min, 15 mg every 12 hr. less than 60 kg: extended-release 37.5 mg once daily;

CrCl 10-25: mL/min. 60 kg or greater: 20 mg every 12 hr. 60 kg or greater extended-release: 50 mg once daily;

CrCl 10-25: mL/min, 50 mg every 48 hr

CrCl 25-10: less than 60 kg 15 mg every 24 hr.

less than 60 kg: extended-release 37.5 mg every 48 hr.

60 kg or greater: 20 mg every 24 hr.

60 kg or greater extended-release: 50 mg every 48 hr.

Important side effects

Pancreatitis and painful peripheral neuropathy. diarrhea, nausea, vomiting, fat redistribution, headache, lactic acidosis and hepatomegaly with steatosis, motor weakness, rash.

Important drug interactions

Didanosine, doxorubicin, hydroxyurea, ribavirin, zidovudine.
