Sparfloxacin (Zagam®)


CrCl Formula

CrCl > 80: 400 mg load then 200 mg po q d.

CrCl 80-50: 200 mg po q 24 h.

CrCl 50-10: 200 mg po q 48 h.

CrCl < 10: 200 mg po q 48 h.

Important side effects

Like all quinolones, can affect the QT interval. All variety of neuropsychiatric side effects; not recommended for children, adolescents, and pregnant or lactating women due to arthropathy; perhaps achillies tendon rupture. Maybe retinal detachment (PubMed).

Important drug interactions

Ca, Mg, Fe, and Sucralfate will markedly inhibit po absorption. acecainide, acetophenazine, aluminum carbonate, basic, aluminum hydroxide, aluminum phosphate, amiodarone, amitriptyline, amoxapine, antidiabetic agents, astemizole, bepridil, bretylium, calcium, chlorpromazine, cisapride, corticosteroids, desipramine, dihydroxyaluminum aminoacetate, dihydroxyaluminum sodium carbonate, disopyramide, dofetilide, dothiepin, doxepin, erythromycin, ethopropazine, fluphenazine, haloperidol, ibutilide, imipramine, iron, lofepramine, magaldrate, magnesium carbonate, magnesium hydroxide, magnesium oxide, magnesium trisilicate, mesoridazine, methotrimeprazine, moricizine, nortriptyline, pentamadine, perphenazine, pipotiazine, pirmenol, prajmaline, probucol, procainamide, prochlorperazine, promazine, promethazine, propiomazine, protriptyline, quinidine, recainam, sotalol, sucralfate, terfenadine, thiethylperazine, thioridazine, trifluoperazine, triflupromazine, trimipramine, zinc, ziprasidone.
