Quinine (Atabrine®)

CrCl Formula CrCl > 80: 650 mg po q 8 h.

Important side effects

Disseminated intravascular coagulation, hemolytic uremic syndrome, hepatotoxicity, hypoglycemia, interstitial nephritis, nausea/vomiting, dysphagia, epigastric pain, ototoxicity (rare, reverses when drug stopped), thrombocytopenia.

Important drug interactions

Astemizole, aurothioglucose, cyclosporine, dicumarol, digoxin, droperidol, mefloquine, metformin, pancuronium, rifampin, rifapentine, ritonavir, saquinavir, warfarin.

Pearls

Check for glucose-6-phosphate dehydrogenase deficiency, which, being protective for malaria, they should not have. G6P that is.
