Ofloxacin

CrCl Formula

CrCl > 80 200 - 400 mg po/iv q 12 h.

CrCl 80-50 200 - 400 mg po/iv q 12 h.

CrCl 50-10 200 - 400 mg po/iv q 12 h.

CrCl < 10 100 - 200 mg po/iv q 24.

Hemodialysis 200 mg load then 100 mg q 24 h.

Important side effects

Diarrhea, nausea, dizziness, headache, peripheral neuropathy, qt interval prolongation (rare), severe hypersensitivity reactions (rare), tendon rupture (rare).

Aluminum carbonate, basic, aluminum hydroxide, aluminum phosphate, antidiabetic agents, calcium, corticosteroids, cyclosporine, dihydroxyaluminum aminoacetate, dihydroxyaluminum sodium carbonate, iron, magaldrate, magnesium carbonate, magnesium hydroxide, magnesium oxide, magnesium trisilicate, nitrofurantoin, sucralfate, theophylline, tizanidine, warfarin, zinc.
