CrCl Formula

CrCl > 80: 400 mg po q 12.

CrCl 80-50: 400 mg po q 12.

CrCl 50-10: 400 mg po q 12.

CrCl < 10: 400 mg po q 12.

Important side effects

Diarrhea, nausea, dizziness, headache, peripheral neuropathy, qt interval prolongation (rare), severe hypersensitivity reactions (rare), tendon rupture (rare).

Important drug interactions

Aluminum carbonate, basic, aluminum hydroxide, aluminum phosphate, antidiabetic agents, calcium, corticosteroids, cyclosporine, dihydroxyaluminum aminoacetate, dihydroxyaluminum sodium carbonate, iron, magaldrate, magnesium carbonate, magnesium hydroxide, magnesium oxide, magnesium trisilicate, nitrofurantoin, sucralfate, theophylline, tizanidine, warfarin, zinc.

Pearls

As with all quinolones, antacids bind the drug to prevent absorption.

Treatment of choice

Use for

UTI and diarrhea only but it has the following indications: Cholera, Conjunctivitis, Enterobacter infections, Escherichia coli infections, Gonorrhea, Klebsiella pneumoniae infections, Malaria, nongonococcal Urethritis, Prostatitis, Proteus, Providencia stuartii infections, Salmonella infections, Serratia infections, Shigella, travelers' Diarrhea prophylaxis, UTI.

Don't use for

Systemic illnesses.
