Nitrofurantoin ( Macrobid®, Macrodantin®)


CrCl Formula

CrCl > 80: 100 mg po q 12 h.

CrCl 80-50: 100 mg po q h.

CrCl 50-10: do not use.

CrCl < 10: do not use.

Important side effects

Retroperitoneal and pulmonary fibrosis. I see no reason to ever use this drug long term, unless you want your ass sued off. There are plenty of other antibiotics available. My advice: do not use long term. anorexia, nausea, vomiting, headache, hemolytic anemia, hepatitis, cholestatic jaundice, hepatic necrosis (rare), peripheral neuropathy (rare, can be severe and irreversible (PubMed)).

Don't use it long term. Ever. Never. Causes retroperitoneal fibrosis and only works if CrCl > 50, which is probably no one over age 65. Hint: CrCl declines with age.
