CrCl > 80: 200 mg po q 12 h.

CrCl 80-50: 200 mg po q 12 h.

CrCl 50-10: 200 mg po q 12 h.

Important side effects

Headache, fatigue, diarrhea, nausea, fat redistribution, hepatitis (there is a genetic predisposition (PubMed) its MDR1 3435C to T if you are interested), hepatic failure, rash (very common can go to Stevens-Johnson syndrome, toxic epidermal necrolysis).

Amiodarone, amprenavir, atazanavir, carbamazepine, caspofungin, cisapride, clarithromycin, clonazepam, cyclophosphamide, cyclosporine, diltiazem, disopyramide, ergotamine, ethinyl estradiol, ethosuximide, etonogestrel, fentanyl, fluconazole, fosamprenavir, indinavir, itraconazole, ketoconazole, levonorgestrel, lidocaine, lopinavir, medroxyprogesterone, mestranol, methadone, nifedipine, norethindrone, norgestrel, quinupristin/dalfopristin, rifabutin, rifampin, rifapentine, saquinavir, sirolimus, St john's wort, tacrolimus, verapamil, voriconazole, warfarin.
