CrCl Formula

CrCl > 80: 1 gm po q 6.

CrCl 80-50: 1 gm po q 6.

CrCl 50-10: 1 gm po q 6.

CrCl < 10: do not use.


Peritoneal dialysis:

Important side effects

Coma, diarrhea, dizziness, drowsiness, headache, hemolytic anemia, metabolic acidosis, n/v, peripheral neuropathy, photosensitivity, psychotic reactions, qt interval prolongation, rash, seizures, tendon rupture.

Important drug interactions

Antacids, cyclosporine, didanosine, disopyramide, melphalan, pirmenol, prajmaline, procainamide, quinidine, sucralfate, warfarin.
