Miconazole (Monistat®)

Mostly used as a topical agent.


CrCl Formula

CrCl > 80: 400-1200 mg iv q 8 h.

CrCl 80-50: 400-1200 mg iv q 8 h.

CrCl 50-10: 400-1200 mg iv q 8 h.

CrCl < 10: 400-1200 mg iv q 8 h.

Hemodialysis:400-1200 mg iv q 8 h.

Peritoneal dialysis: 400-1200 mg iv q 8 h.

Important side effects

Important drug interactions

Acenocoumarol, anisindione, benzodiazepines, cyclosporine, dicumarol, fentanyl, fosphenytoin, glimepiride, phenindione, phenprocoumon, phenytoin, pimozide, tolterodine, trimetrexate, warfarin.

Rants and Screeds

Never used systemically anymore with the newer agents available.

Topically for candida vaginitis. candidiasis - arthritic/mucocutaneous/ vaginitis, coccidioidomycosis, dermatomycosis, diaper dermatitis, petriellidosis, tinea corporis, tinea infections, tinea nigra palmaris, tinea pedis, tinea versicolor, tinea versicolor.
