Meropenem (Merrem®)


CrCl Formulatteint

CrCl > 80: 500 mg - 2 gm iv q 8 h. Give with a 4 hour infusion is probably best in the ill ICU patient.

CrCl 80-50: 500 mg - 1 gm iv 8-12h.

CrCl 50-10: 500 mg - 1 gm iv 0.5-1 g q 12 h.

CrCl < 10: 500 mg - 1 gm iv 0.5 g q 24 h.

Important side effects

Agranulocytosis, neutropenia, (rare), diarrhea, nausea, headache, seizures (usually in patients with cns disorders), toxic epidermal necrolysis, steven-johnson syndrome, erythema multiforme, angioedema.

It may be safer than imipenem in patients with a type 1 drug reaction to penicillin (PubMed).

Treatment of choice

Use for

Damn near anything. Cellulitis, Cystic fibrosis, Diverticulosis/itis, Neutropenic fever, Intra abdominal infections, Meningitis, Nocardia infections, Pneumonia - nosocomial, UTI, ventriculitis.
