CrCl Formula

CrCl > 80: Idoxuridine ophthalmic solution (0.1%), one drop into each infected eye every hour during the day and every 2 hours at night, and continued until a definite improvement has occurred by loss of staining with fluorescein, the decrease to one drop every 2 hours during the day and every 4 hours at night. To decrease recurrence, therapy should be continued at this reduced dosage for 3 to 5 days after healing appears to be complete. Alternatively, one drop may be instilled every minute for five minutes. This schedule should be repeated every four hours, night and day. Treatment should be continued for 3 to 5 days after healing is complete, not continuing for more than 21 days.
