Gemifloxacin (Factive®)

Dosing

CrCl Formula

CrCl > 80: 320 mg po qd.

CrCl < 40: 160 mg po qd.

Hemodialysis: 160 mg po qd.

Peritoneal dialysis: 160 mg po qd.

Important side effects

Beware ever so slightly of patients with long qt (that's an EKG finding), it can make it worse. Diarrhea, nausea, dizziness, headache, hyperglycemia, hypoglycemia (usually in diabetic patients), peripheral neuropathy, tendon rupture (rare), torsades de pointes (rare).

Aluminum carbonate, basic, aluminum hydroxide, aluminum phosphate, antidiabetic agents, antipsychotics, arsenic trioxide, astemizole, bepridil, calcium carbonate, chloral hydrate, chloroquine, cisapride, clarithromycin, class i antiarrhythmic agents, class ia antiarrhythmic agents, class iii antiarrhythmic agents, corticosteroids, didanosine, dihydroxyaluminum aminoacetate, dihydroxyaluminum sodium carbonate, dolasetron, droperidol, erythromycin, fluconazole, fluoxetine, foscarnet, halofantrine, iron, isradipine, levomethadyl, lidoflazine, magaldrate, magnesium carbonate, magnesium hydroxide, magnesium oxide, magnesium trisilicate, mefloquin, mesoridazine, octreotide, pentamadine, phenothiazines, pimozide, probucol, quercetin, spiramycin, sucralfate, sulfamethizole, terfenadine, thioridazine, tricyclic antidepressants, trimethoprim, vasopressin, venlafaxine, zinc, ziprasidone, zolmitriptan.
