CrCl > 50: 500 mg iv q 8.
CrCl ≥30 to ≤50: 250 mg iv q 8 hours.

There may be better outcomes if given as a 4 hour infusion rather than a one hour infusion (PubMed).
Treatment of choice
When compared head to head, it was inferior (more deaths) to imipenam and the trial was stopped early.
Don't use for
Staphylococcus aureus (MRSA), Enterococcus faecium, and Stenotrophomonas maltophilia.
